Student Registration Information
Please keep in mind that our preschool is only for students with special needs. Please call our office if you have any questions.
Enrollment packets may be picked up at the school office.
Office hours are from 7:30 am - 4:00 pm, M-F (except holidays).

The following are needed to complete your child's registration process:
- Parent/Guardian Identification (State ID, Driver License or Passport)
- Proof of Residence (click on this link for more information)
- Legal Documents (if applicable - i.e. custody, divorce, TRO, Power of Attorney)
For students transferring from another Hawaii public school, please also submit:
- Certificate of Release documents from last school (last school may also fax it to us at 808-974-4197)
For students transferring from the mainland or a private school, please also submit:
- Any release documents from the child's last school (including report card if available and health records; last school may also fax it to us at 808-974-4197)
- Child's Birth Certificate (NOT the birth record) or Passport
- Student's Health Record - Form 14 or similar (form available in enrollment packet)
For new students (i.e. Kindergarteners), please also submit:
- Child's Birth Certificate (NOT the birth record) or Passport
- Student's Health Record - Form 14 or similar (form available in enrollment packet)
For students with special needs, please also submit:
- Student's latest IEP or 504 plan
